PARENT CHECKLIST

****Items in bold print are required or your child cannot be registered for camp.****
Notice: You must have a camp physical, sports physical, or well check performed by a health practitioner within the 23 months prior to the beginning date of the camp your child is attending. Please be aware your health care provider may require another exam in order to sign the health form.
Items required by mail:
___Completed Registration Form (May be completed and sent Online)
___All required signatures are completed. (Including the child's signature.)
___My employer's name and phone # are on the form.
___Completed Health Form with physical exam is signed by a health practitioner.
___My family doctor's name and complete address are on the health form.
___A check or money order for at least the pre-registration fee is signed, made out to Tepee Bible Camp, and is enclosed.
ITEMS REQUIRED UPON ARRIVAL AT CAMP
____I have brought the remainder of my child's camp fee with me.
____ I have made sure I have provided the additional money for my child to visit the Trading Post.
____I have made sure my child has all the Items to Bring listed in the camp registration information, including long pants, a hat, extra shoes, & water bottle for the hike.
____I have brought all my child's prescription and over-the-counter medications in their original packaging with their name written on them in permanent ink.
____I have made sure my child has the washable mat to sleep on required by Colorado Division of Childcare.
____I have brought my child's medical papers with me since I did not send them by mail.
____I have checked my child's belongings and there are no items from the DO NOT BRING TO CAMP list.
PARENTAL VISITATION:
Parent visitation time is Friday from 2-5 p.m. during Barnyard Olympics and final awards. If your child becomes homesick you will be notified. Our policy is to encourage them to stay for the entire camp.
REMINDER: KEEP THIS CHECKLIST UNTIL YOU PICK UP YOUR CHILD FROM CAMP.
